Saturday – Sunday 8:00 AM to 1:00 PM (infertility only)
To prepare for your initial visit, please note the following:
- Call your insurance company to determine if you are required to have a referral from your primary care doctor and/or an authorization from your insurance company for your initial visit. Most gynecology visits do not require these; however, some infertility visits do.
- Please bring your insurance card and photo ID (driver's license)
- Please bring any pertinent medical records that you have in your possession. It is okay if you do not have records available; I will determine which, if any, are needed after your initial visit.
- If you have a printer, please print and fill out the following form and bring it to your initial visit.
